چکیده

Amyotrophic lateral sclerosis (ALS) is recognized as a motor neuron disorder affecting the structure and function of the brain and neuromuscular system. Very little is known, however, that the nervous system degeneration is dependent on disease phenotypes of ALS. The purpose of this study was to determine the degree of brain white matter (WM) structure degeneration in four ALS patient groups characterized by their clinical signs and neuroimaging measurements. Fractal dimension (FD) of three-dimensional (3D) brain WM images was quantitatively analyzed to evaluate the WM structural complexity, including complexity levels of the WM skeleton, surface and general structures in ALS patients and control subjects. A total of 100 participants were assigned into five groups: ALS patients with frontotemporal dementia (ALS-FTD, n=20), ALS patients with predominantly upper motor neuron (UMN) signs and hyperintensity MRI signals on corticospinal tract (CST) (UMN-CST + , n=20), ALS patients with predominantly UMN signs but without hyperintensity signal on CST (UMN-CST-, n=27), ALS patients with an equal amount of UMN and lower motor neuron (LMN) signs (ALS-classic, n=22), and a neurological control group (n=11). The brain was extracted

Brain White Matter Shape Changes in Amyotrophic Lateral Sclerosis (ALS): A Fractal Dimension Study

Amyotrophic lateral sclerosis (ALS) is a fatal progressive neurodegenerative disorder. Current diagnosis time is about 12-months due to lack of objective methods. Previous brain white matter voxel based morphometry (VBM) studies in ALS reported inconsistent results. Fractal dimension (FD) has successfully been used to quantify brain WM shape complexity in various neurological disorders and agin...

Integration of structural and functional magnetic resonance imaging in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis as a system failure is a concept supported by the finding of consistent extramotor as well as motor cerebral pathology. The functional correlates of the structural changes detected using advanced magnetic resonance imaging techniques such as diffusion tensor imaging and voxel-based morphometry have not been extensively studied. A group of 25 patients with amyotroph...

Voxel-Based MRI Intensitometry Reveals Extent of Cerebral White Matter Pathology in Amyotrophic Lateral Sclerosis

Amyotrophic lateral sclerosis (ALS) is characterized by progressive loss of upper and lower motor neurons. Advanced MRI techniques such as diffusion tensor imaging have shown great potential in capturing a common white matter pathology. However the sensitivity is variable and diffusion tensor imaging is not yet applicable to the routine clinical environment. Voxel-based morphometry (VBM) has re...

Widespread grey matter pathology dominates the longitudinal cerebral MRI and clinical landscape of amyotrophic lateral sclerosis

Diagnosis, stratification and monitoring of disease progression in amyotrophic lateral sclerosis currently rely on clinical history and examination. The phenotypic heterogeneity of amyotrophic lateral sclerosis, including extramotor cognitive impairments is now well recognized. Candidate biomarkers have shown variable sensitivity and specificity, and studies have been mainly undertaken only cro...

White matter alterations differ in primary lateral sclerosis and amyotrophic lateral sclerosis.

Primary lateral sclerosis is a sporadic disorder characterized by slowly progressive corticospinal dysfunction. Primary lateral sclerosis differs from amyotrophic lateral sclerosis by its lack of lower motor neuron signs and long survival. Few pathological studies have been carried out on patients with primary lateral sclerosis, and the relationship between primary lateral sclerosis and amyotro...
